Choosing between Original Medicare and Medicare Advantage is one of the most important decisions you'll make when enrolling in Medicare. Both options have their pros and cons, and the right choice depends on your individual health needs, budget, and preferences.

What is Original Medicare?

Original Medicare is the traditional fee-for-service program run directly by the federal government. It includes:

  • Part A (Hospital Insurance): Covers inpatient hospital stays, skilled nursing facility care, hospice, and some home health care
  • Part B (Medical Insurance): Covers doctor visits, outpatient care, preventive services, and medical equipment

What is Medicare Advantage?

Medicare Advantage (Part C) is an alternative to Original Medicare offered by private insurance companies approved by Medicare. These plans must cover everything Original Medicare covers, but often include additional benefits.

Key Differences

Network Flexibility

Original Medicare: You can see any doctor or hospital that accepts Medicare—anywhere in the country.

Medicare Advantage: Most plans use networks (HMO, PPO, etc.). You may need referrals to see specialists, and out-of-network care may cost more or not be covered.

Extra Benefits

Original Medicare: Does not include dental, vision, or hearing coverage.

Medicare Advantage: Many plans include dental, vision, hearing, fitness memberships, transportation to appointments, and more.

Out-of-Pocket Costs

Original Medicare: No out-of-pocket maximum. You pay 20% coinsurance for Part B services with no limit.

Medicare Advantage: Plans have annual out-of-pocket maximums, protecting you from catastrophic costs.

Prescription Drug Coverage

Original Medicare: Requires separate Part D plan for drug coverage.

Medicare Advantage: Most plans include prescription drug coverage (MA-PD plans).

Who Should Choose Original Medicare?

  • You travel frequently or live in multiple states
  • You want maximum flexibility in choosing doctors
  • You're willing to purchase a Medigap policy for additional coverage
  • You have specific specialists you want to keep seeing

Who Should Choose Medicare Advantage?

  • You want an all-in-one plan with predictable costs
  • You value extra benefits like dental, vision, and fitness
  • You're comfortable using a network of providers
  • You want protection from high out-of-pocket costs
