Prescription drug costs can be one of the biggest healthcare expenses for Medicare beneficiaries. Choosing the right Part D plan can save you hundreds or even thousands of dollars each year. Here's how to find the best plan for your medications.
Understanding Part D Coverage
Medicare Part D is prescription drug coverage offered through private insurance companies. You can get Part D coverage in two ways:
- A standalone Part D plan (PDP) if you have Original Medicare
- A Medicare Advantage plan that includes drug coverage (MA-PD)
Step 1: Make a Complete Medication List
Before comparing plans, create a comprehensive list of all your medications, including:
- Drug name (generic or brand)
- Dosage
- How often you take it
- Pharmacy you use
Step 2: Check the Formulary
Each Part D plan has a formulary—a list of covered drugs. Plans organize drugs into tiers, with different cost-sharing for each tier:
- Tier 1: Preferred generics (lowest cost)
- Tier 2: Non-preferred generics
- Tier 3: Preferred brand-name drugs
- Tier 4: Non-preferred brand-name drugs
- Tier 5: Specialty drugs (highest cost)
Step 3: Consider Total Costs
Don't just look at the premium! Consider all costs:
- Monthly premium: What you pay each month
- Annual deductible: What you pay before coverage kicks in
- Copays/coinsurance: What you pay for each prescription
- Coverage gap (donut hole): What you pay after initial coverage limit
Step 4: Check Your Pharmacy
Part D plans have preferred pharmacy networks. Using a preferred pharmacy can significantly reduce your costs. Make sure your regular pharmacy is in-network, or consider switching to save money.
Step 5: Watch for Restrictions
Some drugs may have restrictions:
- Prior authorization: Plan must approve before covering
- Step therapy: Must try cheaper drugs first
- Quantity limits: Limits on how much you can get
The Inflation Reduction Act Impact
Recent changes have improved Part D coverage:
- $35/month cap on insulin costs
- Free vaccines covered under Part D
- $2,000 annual out-of-pocket maximum starting in 2025
