Understanding Medicare Enrollment Periods: When Can You Sign Up?

One of the most confusing aspects of Medicare is understanding when you can enroll. Missing your enrollment window can result in penalties and gaps in coverage. Here's what you need to know about Medicare enrollment periods.
Initial Enrollment Period (IEP)
Your Initial Enrollment Period is a 7-month window centered around your 65th birthday:
- Starts 3 months before your birthday month
- Includes your birthday month
- Ends 3 months after your birthday month
This is your primary opportunity to enroll in Medicare Parts A and B without facing late enrollment penalties.
Annual Enrollment Period (AEP)
The Annual Enrollment Period runs from October 15 through December 7 each year. During this time, you can:
- Switch from Original Medicare to a Medicare Advantage plan
- Switch from Medicare Advantage back to Original Medicare
- Change from one Medicare Advantage plan to another
- Join, drop, or switch Part D prescription drug plans
Medicare Advantage Open Enrollment Period
From January 1 through March 31, if you're enrolled in a Medicare Advantage plan, you can:
- Switch to a different Medicare Advantage plan
- Drop your Medicare Advantage plan and return to Original Medicare
- Join a Part D plan if you return to Original Medicare
Special Enrollment Periods (SEP)
Certain life events trigger Special Enrollment Periods, allowing you to make changes outside the normal enrollment windows:
- Moving to a new area
- Losing employer or union coverage
- Qualifying for Medicaid or Extra Help
- Your plan changes its contract with Medicare
Late Enrollment Penalties
If you don't enroll in Medicare when you're first eligible, you may face permanent penalties:
- Part B penalty: 10% added to your premium for each 12-month period you could have had Part B but didn't sign up
- Part D penalty: 1% of the national base premium for each month you went without creditable drug coverage
